Recovery from this is disputed and appears to depend on the amount of time that the penis has been confined. Most reports suggest that it would take years to reach this state and even then, a longish period of ‘normal’ erection activity would restore most, if not all, the previous dimensions – dependant of the age and general state of health.

I did see a TikTok video where a Dr mentioned growers wearing a cage could cause micro fractures as the penis grows and shrinks which may result in PD.
Regarding penis-shrinkage due to the use of chastity cages there is indeed a lot of untrue and unverified information. What I noticed is that people pretending wearing cages for a long time do not mention 4 standard things that happen when you really do wear for an extended period of time and what makes me think that it is not true they wear all the time without taking the chastity cage of:
1- If you do not take the cage of at least once a day to wash your penis and chastity-cage it starts to smell bad already after 1 day even if you are circumcised. The smell coming from the urine between the cage and the penis and the urine under the foreskin. If you a have partner with who you live together it will not take more than 2 days before they will smell it and make remarks. I take it of and on and wash it twice a day, in the morning and in the evening.
2 - Peeing is quite a problem. Sometimes you can pee standing without spilling but it is impossible that this works every time you pee. A lot of times you will pee all around and having your complete cage also wet at the outside and mostly also wet your clothes. You can dry it off with toilet paper but if you do not wash the thin film of left urine it will start to smell in less then a day. An urethral tube in the cage could be the solution but not for an extended time. You will get UTI’s in less than a week especially if the tube is not sterilized all the time, lubed and inserted with sterile gloves. So the solution is peeing sitting as a girl and you can then dry of the end of the cage with toilet paper. However you can not get the last drops of urine
out of the last part of the penis because it is trapped in the chastity cage and you can not make that brief wanking action that all men do to get the last drops out not to wet your underwear. So most of the time just after peeing or some time later more then a few drops will come out of your penis and wet your underwear.
And if you are not lucky it will wet also through your jeans. So if you really want to wear for extended periods then more the a few hours and be having a normal life you will have to wear a kind of pads in your underwear or wear a kind of briefs for light incontinence. I do wear special underwear.
3 - Only a few talk about true hygiene being washing your penis with fungal soap to avoid fungal infections and the use of oil to prevent skin rash so the others had no problem with this what is impossible. I practice this hygiene.
4: I read a few times in hundreds of posts about people pretending to be in chastity for long periods that they had a testicular torsion or an epididymitis, things that are likely to happen if you are in chastity for many years being the inflammation of the epididymis the side part of the testicles that stores the spermcells before ejaculation or a twisted testicle-cord. This is very painful. I had several times an epididymitis and it is caused because of the ring of the chastity device being to tight or by to much pressure on the testicles. You can have it even with the perfect ring but with to much pressure in a certain position on a part of the testicles what happens easily when you sleep without noticing it. Because I had it several times and that my girlfriend insisted already before that that I would have a vasectomy I had an epididydectomy to remove both of them to solve the problem.
Anyhow I am already 13 years most of the time in chastity (Holytrainer V5) and it is quite ok taking into account the above measures. It gives the impression that it is maybe not worth all that fuss but my girlfriend and myself love it and our cuckolding relationship is very rewarding for us and makes us happy.

So long-term penis atrophy (aka shrinkage) is likely due to this reduction in the frequency of nocturnal erections. And it is to expect that you will have fewer erections anyhow when you are on HRT. So It can be confirmed that its medically accepted in the HRT-world that “if you do not use it you will loose it” and that people that are in 24/7 chastity for a long time indeed will get ED and shrinkage…
As someone who is often in extended periods of chastity, your summary is spot on. Peeing can be hard. It goes everywhere except down and this can be a problem I have to sit to pee and it flies out in all directions.
Yes it will smell quickly even with clean as you will leak urine. I notice I leak a lot of pre cum as well.
